On the day of your appointment
We ask that you arrive 10 minutes before the start so that our staff can prepare you for your visit and update your personal and insurance information. We also recommend that you always carry a listing of your current medications, dose, and frequency. If you're a new patient, please bring your previous doctor's information (name, address, phone #). Check with your insurance company, as you may be required to pay your deductible or co-payment. If this is your very first appointment, please bring your current insurance card, identification, and fill out the
patient registration form.

How to get a medication refill
Please contact your pharmacy. They will send a request to your doctor to approve the refill. Please allow 3 business days for a routine refill request. If you are out of medication, please let the pharmacist know so that they can expedite the request.

To receive copy of your (or your children's) medical records, you must first sign a
release of information form.

How to access Case Managers
Sutter Medical Foundation North Bay employs registered nurse case managers to assist members experiencing catastrophic illness or critical health events. Case managers assist their clients with navigation of the health care system in order to facilitate regaining optimum health or function. To refer yourself or a family member, phone your Primary Care Physician or Sutter Medical Foundation North Bay at (707) 521-8830.

How to access Utilization Management Guidelines
Sutter Medical Group of the Redwoods makes all decisions regarding requests for medical care based on the medical necessity/appropriateness of the request and/or the member's benefit coverage. Clinical guidelines, affirmative statements, and answers to specific utilization management questions are available by calling our toll-free customer service number between 9 AM and 5 PM Monday through Friday at 800-470-0071. Our customer service representative will connect you with the appropriate clinical service team member.
